Case Study: How a Large FQHC passed their On-Site Visit with ZERO findings

The struggle: Their checklist was unclear and did not list all of the policies. (The policy said one thing, checklist another.) All policies and procedures need to be on the checklist since the organization MUST be doing everything on their policy and procedures.


The solution: Our PSV specialist and HR Team revamped the client’s checklist so that it matched their policies and procedures. Specifically, on the pre-on-site visit, the auditor discovered the immunization policy didn’t include important details like provider declinations. The revamped immunization attestation form spelled out all required immunizations with an area to decline individual immunization — so it was in writing and signed by the provider and the client had backup to support the policy.


The result: This was the client’s first audit since outsourcing to InteCare, and it was completed without dings. The large California FQHC passed with flying colors!

  • Who conducts your HRSA and other accreditation agency audits?

  • When can auditors come to your facility or request information?

    Healthcare auditors can come at any time to conduct audits. Their On-site Visit (OSV) are typically scheduled in advance, but they may also perform surprise audits to ensure compliance and accuracy of records. Healthcare organizations need to be prepared and maintain accurate documentation at all times to facilitate a smooth audit process.

  • What happens during the audit?

    During a healthcare audit, various aspects of the organization's operations may be examined, including provider credentialing and privileging, medical records, billing and coding practices,  adherence to legal and regulatory requirements, infection control procedures, medication management, and overall patient care. Auditors typically review documentation, interview staff members, observe processes, and analyze data to evaluate the organization's performance.

  • What happens if your policies and procedures don’t match up?

    If an auditor comes in and reviews your policies and procedures. For example: your employee policy may read “TB test required every 12 months” but in practice, the organization switched to a  questionnaire. Since they changed what they collect but didn’t change it in policy and procedure. They’ll get “deemed” or dinged. Getting dinged doesn’t mean you’ve failed; however, there is corrective action required. And after following through with the corrective actions, auditors might come back to the site or virtually check anywhere right away or 2 years later at the next audit.
